What to bring to your first visit?
We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite the first appointment, we ask that you please bring your child's immunization records and be prepared to complete paperwork regarding your family medical history, your child's medical history, and insurance information.
We are also happy to offer you the option to download, print and complete our office forms to reduce wait times during your first appointment.

What to Expect
On the day of your child's first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. We look forward to meeting you and joining with you to help your child maintain optimal health.
Policies and Fees
At your first visit and every year, you will have a chance to review our HIPAA and Financial Policies. Of course, you can ask to see these or ask any questions throughout the year.
Some services that we perform require a fee.
Forms and Prescriptions: We will complete your forms and prescription refills for FREE if you give us 5-7 business days. We can Rush your form or prescription for $12.00 or provide them Urgently within 24 hours for $25.00.
No Show Appointment: without 24-hour notice, a $25.00 charge will be required, After 3 no-show appointments, we will consider dismissal from our office
Co-Payment not paid: If not paid at the time of service or within 2 days after the date of service, $10.00
Billing fee: a fee of $10.00 will be charged per month for each statement sent for the balance after the first statement.
Termination fee: if we turn your account to collections, a fee of $25.00. Anyone who transfers out of our office with a balance will automatically be sent to collections.
Insufficient Funds (Bad Check): $50.00
